What is Pirfenidone?

Pirfenidone is a medication prescribed to manage idiopathic pulmonary fibrosis (IPF), a chronic and progressive lung disease. IPF causes scarring deep within the lungs, leading to thickened, stiff lung tissue. This makes breathing difficult and can impact oxygen flow to the brain, heart, and other vital organs. While the exact cause of IPF is often unknown, it's a serious condition that can be life-threatening. Pirfenidone itself doesn't cure IPF, but it can help slow the disease's progression.

It's important to understand that pirfenidone is a treatment option, not a solution. It works by modulating the body's inflammatory and fibrotic responses in the lungs, aiming to reduce the rate of scarring. The medication may also be used for other purposes as determined by a physician.

Before Taking Pirfenidone:

You shouldn't take pirfenidone if you're allergic to it. Before starting treatment, let your doctor know if you have:

Pregnancy and Breastfeeding: The effects of pirfenidone on unborn babies and nursing infants aren't fully understood. Inform your doctor if you're pregnant, planning to become pregnant, or breastfeeding.

How to Take Pirfenidone

Your doctor will conduct blood tests to ensure pirfenidone is safe for you. Adhere strictly to the dosage instructions on your prescription label. Never adjust the dosage without consulting your physician.

Take pirfenidone with food at the same time each day. The usual dosage involves three times daily administration, but the initial dosage is carefully managed:

  • Week 1: 1 capsule or tablet per dose
  • Week 2: 2 capsules or tablets per dose
  • Week 3 onwards: 3 capsules or tablets per dose (or 1 801mg tablet per dose, three times daily)

Important: This gradual increase in dosage (dose escalation) is crucial. You must follow this schedule when starting treatment or restarting after a break of 14 days or more. Do not exceed the recommended maximum daily dose of three administrations. If you stop taking pirfenidone for any reason, consult your doctor before restarting.

Regular blood tests will monitor for potential liver-related side effects. Test results may lead to temporary dosage adjustments or delays.

Store pirfenidone at room temperature, away from moisture and heat. Keep the container tightly closed.

Missed Dose

Take the missed dose as soon as you remember, unless it's nearly time for your next scheduled dose. Never double up on doses.

What to Avoid While Taking Pirfenidone

Minimize sun exposure. Pirfenidone increases sun sensitivity, so wear protective clothing and use a high SPF sunscreen (SPF 30 or higher) when outdoors. Avoid tanning beds.

Inform your doctor if you're using antibiotics, as they can also enhance sun sensitivity. Also, avoid smoking, as it can reduce pirfenidone's effectiveness.

Pirfenidone Side Effects

Seek immediate medical attention if you experience signs of an allergic reaction: hives, difficulty breathing, swelling of the face, lips, tongue, or throat.

Contact your doctor immediately if you experience:

  • Severe or persistent stomach pain, vomiting, or diarrhea
  • Burning or pain in your esophagus or throat
  • Signs of liver problems: upper right abdominal pain, easy bruising or bleeding, fatigue, dark urine, clay-colored stools, jaundice (yellowing of skin or eyes)

Common side effects include:

  • Nausea, vomiting, loss of appetite
  • Stomach pain, heartburn, indigestion
  • Diarrhea
  • Headache, dizziness, fatigue
  • Weight loss
  • Cold symptoms (stuffy nose, sneezing, sore throat)
  • Joint pain
  • Sleep problems (insomnia)
